Veterans' Health Assessment — Template & Note Generator
Work through the regulated history and examination domains, add your findings, and generate a clean, structured note to paste into your clinical software. For the one-off MBS Veterans' Health Assessment (items 701/703/705/707) for former ADF members.
📋 Source: MBS health assessment items 701–707; DVA arrangements — MBS Online (mbsonline.gov.au) & Department of Veterans’ Affairs. Verified June 2026.
Your doctor or healthcare provider knows your history and circumstances — always follow their personalised advice over general information.
Sources & clinical disclaimer
- The required assessment matters are set out in clause 2.15.10 of the Health Insurance (General Medical Services Table) Regulations 2021, summarised in MBS explanatory note AN.0.69 (Veterans' Health Assessment). MBS Book, July 2026. mbsonline.gov.au.
- Items 701, 703, 705, 707 descriptors and fees — MBS Book, July 2026.
- ADF Post-discharge GP Health Assessment Tool, Department of Veterans' Affairs. dva.gov.au.
Documentation aid and billing reference only — not clinical advice, and it does not capture or transmit patient data. Confirm item descriptors, the full regulated requirements and current fees against the MBS before claiming. Fees are indexed annually on 1 July. Current as at July 2026 MBS.
How this works
The domains below mirror the matters the assessment must include under the regulations. Tick the ones you cover, type findings if you wish, then hit Build note for a copy-paste summary. Everything runs in your browser; nothing is sent or stored. A documentation aid, not clinical advice. The DVA Post-discharge GP Health Assessment Tool is the official template — this is a quick note-builder alongside it.
Billing & eligibility
Eligible patient: a former member of the Permanent Forces or a former member of the Reserves. One-off — claimable once.
Usual doctor rule: must be performed by the patient's usual GP or prescribed medical practitioner (one providing, or anticipated to provide, at least 50% of the patient's primary care). This requirement is stricter than for health assessments generally.
Item is set by time spent — use the time-tiered item matching the duration.
| Item | Time | Fee (100% rebate) |
| 701 | Brief — not more than 30 min | $71.00 |
| 703 | Standard — 30 to under 45 min | $165.05 |
| 705 | Long — 45 to under 60 min | $227.75 |
| 707 | Prolonged — 60 min or more | $321.75 |
Co-claiming: a separate consultation must not be claimed with the assessment unless clinically necessary; any co-claimed item must be a clinically necessary, distinct service.
Not the same as the DVA Annual Veterans' Health Check for certain Veteran Card holders — that is arranged through DVA, not the MBS.
