Schedule fee $87.10 for item 699 (GP), $69.70 for item 177 (PMP). Benefit 100% of schedule fee. EMSN cap: 300% of the schedule fee, to a maximum of $500 — $261.30 (699) / $209.10 (177).
| Criterion | Requirement |
|---|---|
| Minimum time | 20 minutes face-to-face at consulting rooms |
| Patient eligibility | Medicare-eligible AND aged 30 years or over AND would benefit from CVD risk assessment. Intent: identify CVD in people not known to have it (people with known CVD also covered). |
| Frequency | Once per 12 months (per patient) |
| Setting exclusion | Not available to inpatients of hospital, or care recipients in residential aged care facility |
| 12-month exclusion | Cannot be claimed if patient had another health assessment in the previous 12 months — except 715 / 228 / 92004 / 92011 (Aboriginal & Torres Strait Islander Peoples Health Assessment), which doesn't count |
| Usual GP rule | Should be provided by patient's usual GP (or PMP in their practice) where reasonably practicable |
| Practice nurse | May assist (information collection, providing information at the GP's direction). Separate nurse follow-up items have their own conditions: 10997 needs a GP chronic condition management plan (or equivalent) already in place; 10987 is for an Indigenous person who has received a health check (MBS data file, 1 Nov 2026). |
| Record-keeping | Copy of heart health assessment must be retained for 2 years from date of service |

Enter the date of the patient's last health assessment and which item was billed. The calculator applies the 12-month exclusion (with the 715 exception built in) and shows the next eligible date for item 699.
Logic: per MBS explanatory note AN.14.2, item 699 cannot be claimed if the patient has had a health assessment service in the previous 12 months. The 715-family items (715, 228, 92004, 92011) are the only exception — they do not block subsequent 699 claims. Calculator assumes today's date; verify against patient's Medicare claim history before billing.
The MBS descriptor requires all five components plus the minimum time. Tick as you complete them — useful mid-consult or as a note-template prompt.

The MBS explanatory note AN.14.2 explicitly references the Australian Absolute Cardiovascular Disease Risk Calculator at cvdcheck.org.au/calculator.
Inputs: age, sex, ethnicity (Indigenous/non-Indigenous), smoking, systolic BP, total cholesterol, HDL, diabetes status, CKD, AF, LVH. Output: 5-year absolute risk of CV event.
For Aboriginal and Torres Strait Islander patients, the MBS explanatory note also references the RACGP National Guide to preventive healthcare for Aboriginal and Torres Strait Islander people.
| Scenario | Allowed? |
|---|---|
| 699 + standard consult (23/36/44) same visit | Not generally — a separate consultation alongside the heart health assessment is only allowed if clinically necessary (acute issue requiring separate management). Document the clinical justification. |
| 699 + 715 within 12 months (either order) | Yes — the 715-family items (715/228/92004/92011) are explicitly excluded from the 12-month rule. A patient can have both in the same 12 months. |
| 699 + another non-715 health assessment within 12 months | No — 699 cannot be claimed if the patient has had another health assessment (other than 715-family) in the previous 12 months. |
| 699 + 695 (Menopause) within 12 months | No — 695 is a health assessment item, so it falls under the 12-month exclusion. Choose which to claim first; the other becomes available 12 months later. |
| 699 + Bulk Billing Incentive (10990 / 75870 etc.) | Yes — Medicare bulk billing incentive items can be claimed in conjunction with 699 where the patient is eligible and conditions are met. |
| 699 in hospital outpatient or residential aged care | No — settings explicitly excluded from item descriptor. |
