What item 92064 is used for

A phone attendance by a prescribed medical practitioner to review a GP chronic condition management plan (GPCCMP), prepared by the same practitioner or an associated medical practitioner. It is the telephone counterpart of the video item 92061 and the face-to-face item 393, at the same schedule fee.

What changes on 1 November 2026

Item 92064 is new on 1 November 2026. Until then a plan can be reviewd face-to-face (393) or by video (92061). The MBS Online November 2026 news page lists four new phone items for chronic condition management: 92033 and 92034 (GP), and 92063 and 92064 (prescribed medical practitioner).

What conditions apply to the phone items?

The patient must have a chronic condition and must not be a care recipient in residential aged care. If the patient is enrolled in MyMedicare the service must be provided at the enrolled practice; otherwise by the patient's usual medical practitioner.
