What Item 965 is used for

The initial plan for a patient with a chronic (long-term) condition. Replaced the old GPMP (item 721) and Team Care Arrangement (item 723) on 1 July 2025. Enables the patient to access up to 5 subsidised allied-health visits per calendar year.
