A 20-minute heart health assessment by a GP at consulting rooms for a patient aged 30 or over who would benefit from a cardiovascular risk assessment. It must include collection of relevant information (including a patient history), a basic physical examination that must include recording blood pressure and cholesterol, initiating interventions and referrals as indicated, implementing a management plan, and providing preventative health care advice and information.
Rules from explanatory note AN.14.2: claimable once per patient in a 12-month period; not claimable if the patient has had another health assessment in the previous 12 months, other than an Aboriginal and Torres Strait Islander health assessment (715, 228, 92004, 92011); not available to hospital in-patients or residential aged care recipients; should generally be provided by the patient’s usual doctor; a copy must be kept for two years; practice nurses and Aboriginal and Torres Strait Islander health practitioners may assist with information collection under supervision; bulk billing incentive items can be claimed. The intention is to identify cardiovascular disease in people not known to have it.
