In one line
Both are billed with the time-tiered items 701/703/705/707 (the item is set by time spent, not which assessment). The diabetes risk evaluation is once every 3 years and needs a high AUSDRISK score completed within 3 months; the 45–49 assessment is once only.

Type 2 diabetes risk evaluation (AN.0.37)
Eligible: Medicare-eligible patients aged 40–49 (inclusive) with a high risk of developing type 2 diabetes as determined by the AUSDRISK tool.
AUSDRISK rule: the patient must have completed the AUSDRISK tool no more than 3 months before the evaluation. The service includes reviewing the risk factors behind the high-risk score.
Frequency: cannot be claimed more than once every 3 years for an eligible patient.
When is the diabetes evaluation next claimable?
45–49 chronic-disease-risk assessment (AN.0.38)
Eligible: patients aged 45–49 (inclusive) who are at risk of developing a chronic disease.
Frequency: once only to an eligible patient.
Co-claiming & practical notes
A patient may be eligible for more than one type of health assessment over time (for example a 45–49 patient who is also at high diabetes risk). Each is a separate service billed with the time-tiered item matching the time spent — do not claim two health-assessment items for the one assessment.
The diabetes risk evaluation and the 45–49 assessment overlap at ages 45–49. Document which assessment you are performing and that the eligibility criteria (age, AUSDRISK score and timing, or chronic-disease risk) are met.
These items are not available to admitted hospital patients or residential aged care residents (separate items apply there).
