Number each lesion on the body map and use the same number in the documentation table. This makes it easy to cross-reference locations with clinical details.
For dermoscopy findings, consider noting key features: pigment network, dots/globules, streaks, blue-white veil, regression structures, vascular patterns. Use the 3-point checklist or 7-point checklist as your framework.
This form is designed to fit on a single A4 page when printed in portrait orientation. For patients with many lesions, print multiple copies.
