Find Your Primary Care Network — Canada

Alberta's PCN model is the most established in Canada. There are 39 PCNs representing over 4,200 family physicians and 1,600 allied health providers, serving over 4 million Albertans. PCNs are partnerships between physicians and Alberta Health Services, funded through capitation. They provide team-based care including nurses, dietitians, mental health therapists, social workers, and pharmacists.

BC is establishing PCNs across the province, delivering team-based care through family doctors, nurse practitioners, and allied health. BC also has Divisions of Family Practice — community-based groups of family physicians working together on local health priorities. Urgent and Primary Care Centres (UPCCs) provide same-day access for patients without a family doctor.

Ontario has multiple primary care models. Family Health Teams (over 200) are interprofessional teams led by family physicians. Ontario Health Teams (over 50) are newer, broader integrated health networks that connect primary care with hospitals and community services. The government's Primary Care Action Plan (2025) commits $1.8 billion to connect 2 million more people to primary care. Community Health Centres serve underserved populations.

Quebec's GMFs are groups of family physicians (typically 6–10) who work with nurses, social workers, and other professionals to deliver comprehensive primary care. There are over 400 GMFs serving the majority of Quebec's population. Access is coordinated through local CLSCs (Centres locaux de services communautaires). GMF-Réseau is an expanded model that integrates with specialist and hospital services.

Saskatchewan has primary health care teams typically comprising physicians, nurse practitioners, and allied health. Teams are organised through the Saskatchewan Health Authority. Community health centres provide services in rural and underserved areas. Healthline 811 provides 24/7 health advice.

Manitoba is implementing My Health Teams to connect primary care providers across regions. Community health centres and Access Centres provide team-based care. The Family Doctor Finder service helps unattached patients connect with a physician.

Nova Scotia funds collaborative family practice teams that include nurses, social workers, dietitians, and pharmacists alongside family physicians. The Need a Family Practice Registry connects unattached patients to available practices.

New Brunswick delivers primary care through community health centres and family medicine clinics. Patient Connect NB helps residents without a family doctor get connected. Extra-mural nursing provides home-based primary care support.

PEI is building Patient's Medical Homes — team-based care centred on family physicians. Health PEI manages the Patient Registry Program for residents seeking a family doctor.

Primary care is delivered through regional health authorities and community clinics. Newfoundland has invested in virtual care platforms to improve access, especially in rural areas. The Find a Doctor registry helps connect unattached patients.

In the Yukon, Northwest Territories, and Nunavut, primary care is delivered through community health centres staffed by nurses and visiting physicians. Telehealth is widely used to connect remote communities with specialists. Each territory has a small number of family physicians concentrated in capital cities (Whitehorse, Yellowknife, Iqaluit).
