How do I find out what I'm eligible for from Medicare?

Your Medicare entitlements depend on your age, sex, health conditions, and risk factors. Australians may be eligible for free health assessments (at ages 45-49, 75+, or if Aboriginal/Torres Strait Islander), the Healthy Heart Check (age 30+), free cancer screening (bowel, breast, cervical), GP Management Plans with subsidised allied health visits, Mental Health Care Plans for psychology sessions, and funded vaccines. Ask your GP what applies to you.

What is a chronic disease management plan?

A GP Chronic Condition Management Plan (GPCCMP) is for patients with a chronic condition lasting 6 months or more. It gives you access to up to 5 Medicare-subsidised allied health visits per calendar year — physiotherapy, podiatry, dietetics, psychology, exercise physiology, and more. The old GP Management Plan (GPMP, item 721) and Team Care Arrangement (TCA, item 723) were replaced by the GP Chronic Condition Management Plan (GPCCMP, items 965 and 967) from 1 July 2025. Your GP creates the plan at no cost to you — ask if you qualify.
