A structured screen across physical recovery, mental health, feeding, social risk and baby wellbeing — for midwives and GPs at a postnatal visit or discharge. Fill it in, it saves as you go, then generate a clean summary to print, copy or hand over.
🇦🇺 AustraliaRANZCOG / RACGP-informed
📋 Source structure: RANZCOG postnatal care guidance, RACGP Red Book (postnatal check), PANDA (Perinatal Anxiety & Depression Australia) perinatal mental health resources. This is a documentation prompt list, not a validated clinical scoring instrument — for a validated mood screen, use the EPDS calculator separately.
⚠️ For midwives, GPs and other perinatal clinicians. This is a structured documentation tool — it prompts what to check and lets you flag items you want to escalate. It does not calculate a risk score, diagnose anything, or replace your clinical judgement or your service's own screening protocols (e.g. validated EPDS administration, family violence screening frameworks). Not medical advice.

🩺 You flagged one or more items above. If this warrants a formal handover, the Midwife → GP Postpartum Referral tool can turn this into a structured referral letter.
Frequently asked questions
Does this tool calculate a risk score?
No. It's a structured prompt list — it reflects back the items you've personally ticked as a "flagged" summary, so nothing is missed on a busy day. It doesn't weigh, score, or classify risk itself. The clinical interpretation and any escalation decision is entirely yours.
Should I use this instead of the EPDS?
No — this checklist is not a substitute for a validated screening instrument. If your service protocol calls for a formal mood screen, use the Edinburgh Postnatal Depression Scale (EPDS) as normal. This tool is for the broader structured screen around it — physical, social, feeding, baby — not a replacement for validated tools.
Where is my data stored?
Only in this browser, on this device, via localStorage. Nothing is sent to AskMyGP or anywhere else. If you're on a shared or public computer, use Clear form when you're done, and rely on Print/Copy to get the summary into your actual clinical record system.
What do I do if something is flagged?
Follow your own clinical judgement and your service's escalation pathway — this tool doesn't tell you what to do next. For anything urgent (heavy bleeding, thoughts of self-harm, signs of psychosis, safety concerns), that generally means same-day escalation regardless of what else is on the form. For non-urgent flags, the postpartum GP referral tool can help structure the handover.
