Organised it, or just attended? Community, RACF or discharge? How long did it run? Answer three quick questions and get the exact MBS item number, Schedule Fee, and the co-claiming exclusions that catch people out.
The confusion: Case conferencing has six different GP item numbers (735, 739, 743, 747, 750, 758) that all cover the same three settings — community, residential aged care, and hospital discharge. The item that applies depends on two things only: whether you organised the conference or just participated, and how long it ran.
The catch: you can only claim one role per conference — organising already includes participating, so the organiser can't also bill a participate item for the same meeting. And the organise items can't be co-claimed with a same-day GP Chronic Condition Management Plan or health assessment for the same patient.
Not the same numbers as mental health case conferencing — patients on a GP Mental Health Treatment Plan use a separate item series (930/933/935 organise; 937/943/945 participate), with identical fees to the general series. The picker below covers both — just flip the first toggle.
| Role | Duration | Item | Schedule fee |
|---|---|---|---|
| Organise & coordinate | 15–20 min | 735 | $84.65 |
| Organise & coordinate | 20–40 min | 739 | $144.70 |
| Organise & coordinate | 40+ min | 743 | $241.25 |
| Participate | 15–20 min | 747 | $62.20 |
| Participate | 20–40 min | 750 | $106.60 |
| Participate | 40+ min | 758 | $177.35 |

The organise items (735/739/743) are explicitly excluded, on the same service, from being claimed alongside items 729, 731, 965, 967 (GP Chronic Condition Management Plan items), 231, 232, 392, 393, and the telehealth/video variants 92026, 92027, 92029, 92030, 92057, 92058, 92060, 92061.
In practice, the case this catches most often: preparing or reviewing a GPCCMP and holding a case conference for the same patient on the same day — bill the case conference item, or the care-plan item, not both as the descriptor's own exclusion.
No. "Organise and coordinate" already includes participating in the conference you organised — there's one item per practitioner per conference, chosen by whichever role that practitioner actually performed. If you organised it, bill the organise item only.
No — that's the most common point of confusion. Items 735/739/743 (organise) and 747/750/758 (participate) each cover all three settings: a community case conference, a multidisciplinary case conference for a care recipient in a residential aged care facility, and a multidisciplinary discharge case conference. Only the role (organise vs participate) and the duration change which item applies — not the setting.
Yes — genuinely separate item numbers, same fees. For a case conference concerning a patient's GP Mental Health Treatment Plan, GPs use a different series: 930, 933, 935 to organise/coordinate, and 937, 943, 945 to participate (with parallel series for other medical practitioners, psychiatrists and paediatricians) — set the toggle above to "mental health treatment plan" to use the picker for these. Confirmed against the MBS Mental Health Case Conferencing factsheet and the MBS Book: the Schedule Fees for each duration tier are identical to the general series (735/739/743/747/750/758) — only the item numbers differ.
No. Consultant physicians and specialists organising or participating in a case conference bill from their own, separate item series (in the 800s, e.g. item 820 for a community case conference organised by a consultant physician) — not the GP series 735–758 this picker covers. If you're a GP and a specialist is simply attending the same conference you organised, you still bill your own GP item for your role; the specialist bills theirs separately.
Case conference items are billed by the practitioner to Medicare, not to the patient directly — most practices bulk-bill these. Whether a specific claim is bulk-billed or billed privately (with the patient claiming the rebate) is a decision your practice makes per patient, the same as any other MBS item.
