How to use: Fill in each day the morning after. Rate your sleep quality and daytime sleepiness from 1 (very poor/very sleepy) to 5 (excellent/fully alert). Include any medications, alcohol, or notable events in the notes. After 7 days, tap Print to bring to your appointment.
